Steps when requesting a consult:

1. Simply email (info@star-medical.net) or fax (800-301-9488)  your physician order or prescription for a dietitian consult.
2. We contact the patient to schedule an appointment
3. We report back to the doctors office with our recommendations

For individuals who are not clients of Star Medical: There is no charge for the first consultation, any follow-up consultations will incur a charge.
